Disappearing cover images

hmm, its weird. I'll try to make a list of actions i did before saving 
corrupted file. It could help you to duplicate the problem.

I dont understand the purpose of the flag You proposed. 

I thought about debug messages telling:
1. during save - how many images was saved
2. How many images are referenced in xml file
3. How many references are invalid (without images)

It could help to isolate actions responsible for the corruption. (I could save 
file after every action or set of actions - and look for any anomalies)
